This is a perfect example of how difficult it is to properly manage and test the design of highly complex systems. And as soon as a company's profit motive get's in the way, and forces a more aggressive timeline, the overall design suffers and the quality/safety take a back seat.

What I do not see in this article, is any explanation for the removal of the second type of sensor, which lead to a lot of ill-informed design decisions and ultimately failure and loss of life. Why was it removed? Was the sensor costly and someone tried to save money? (I doubt this is the explanation.)

The way the article is written, make's it sound like a design decision that no one understood. But clearly, a design process that allows for a design change that creates a single point of failure, is an unacceptable process.
